Police Plus Application Privacy Notice (APP)

Thank you for choosing this APP. Please pay attention and read terms and condition below carefully before using this APP.

**Use of this APP is according voluntary and does not require for accessing the Police general hospital related service.

**This is not a contact tracing APP.

Registration

• In using service you will need to use a mobile phone number to register.
• One phone number can use for many patients in same family or relations, but you need to log out and log in for changing.
• One patient need to have only one registered phone number.
• In case of changing phone number, please contact Police general hospital, Medical records division ***.

Identification in the system

• During using this APP. First, you are “guest” can using limited services. (Donating, General information, some profile edition in mobile and News)
• You need to register in mobile app, and verify yourself at Police general hospital, Medical records division for access full function in this APP. (Appointment, verification, self-health data, queue viewer, payment and “guest” function, in early time some functions maybe in developing process).
• You need to give consent at Police general hospital, Medical records division for your health data transferring to this APP, and other hospitals which ready to connect.
• While you are in the registration phase, the system need to verify by sending short messages (SMS) to the registered mobile phone number, so please carefully attention on given correct phone number.

Collected Data

Police Plus (APP), The Police general hospital regarding collected your data according with PDPA AD2019 (Thai personal data protection Act’s AD 2019)

Your demographic data

The citizen ID is especially important for correctly matching and identified user, whereas if you are foreigners we use passport and nation or use citizen-worker ID instead.

• Name, last name, gender, prepay, phone number, hospital ID, are using in purpose to correctly identify patients.
• The second-ID behind your citizen ID card is input to verify your identity with the ministry of interior database via drip chip from personal ID card only first time present at Police general hospital, Medical records division. We are not collect both ID in APP and discard them at the end of the process. This step is crucially important to prevent identity fraud.

The APP is not collect your location, phone, contact or health sensor, only gather data and show time to time.

Again, your health data need to consent at least one time at Police general hospital, Medical records division to activated opening in your mobile via register phone number either or transferring your health data between Police general hospital and others hospital within contact to Police general hospital campaign (that presented in consent form).

The APP need permission to access mobile notifications on the app (push notification) to be alert to various information and services from the system. If not, you need to know some mobile services function maybe interfere you from our hospital service process, such as missing you queue.

All data that we are present on your mobile phone during you using application function, you need to self-protected for your data leakage and data fraud. Police Plus (APP), The Police general hospital is not responsible for your data fraud and data leakage from you or other violated.
